Understanding Your Rehabilitation Journey

Rehabilitation is a personal journey. Understanding how the process works can make it easier to navigate. At PCVRS (Partners in Canadian Veterans Rehabilitation Services), we are committed to supporting you as you work toward your rehabilitation goals and improved well-being. This article explains the roles of PCVRS, Veterans Affairs Canada (VAC), and your rehabilitation team, including how they work together to support your rehabilitation plan.

Navigating Your Path

We know that rehabilitation can feel overwhelming at times. Our goal is to make your rehabilitation journey as simple and clear as possible so you can focus on your health and well-being.

How We Support You:

  • A Dedicated Rehabilitation Service Specialist (RSS): Your RSS is your main point of contact within PCVRS. They can answer your questions, coordinate services and assessments, facilitate communication with your interprofessional care team, and support you throughout your journey.
  • Help Coordinating Your Services: We work with you, your Rehabilitation Service Professionals (RSPs), and VAC Case Manager to support the implementation of your Rehabilitation Plan and keep services focused on your rehabilitation goals. We make sure everyone has the information they need to properly support you.
  • Support When  Issues Arise: Should any challenges arise, your RSS is there to help. They can answer questions, discuss options, and work with you to find solutions.

Supporting Your Rehabilitation

Your Rehabilitation Plan reflects your individual rehabilitation needs and goals. We are dedicated to ensuring that your rehabilitation journey is effective, adaptive, and personalized.

Our Approach to Quality Care:

  • Interprofessional Model: Your rehabilitation team is made up of qualified rehabilitation health and vocational professionals who work together to support your individual rehabilitation needs and goals.
  • Provider Network: Rehabilitation Service Professionals (RSPs) are required to meet high standards of expertise and complete VAC-approved training before providing services.
  • Clinical Oversight Framework: We provide support to RSPs and monitor service delivery to help ensure participants get appropriate care.
  • Tracking Progress & Gathering Feedback: Your rehabilitation team actively monitors your progress through Outcome Measures and gathers your feedback via Participant Experience Check-in Surveys. Together with other information about your rehabilitation, these tools help identify what is working well and opportunities to improve services.

How VAC and PCVRS Work Together

Understanding how VAC and PCVRS work together is key to understanding the program's structure.

Our Partnership:

  • VAC is responsible for the Rehabilitation Program, establishes program policy, determines eligibility, approves Rehabilitation Plans and makes program decisions.
  • PCVRS coordinates and administers rehabilitation services on behalf of VAC, connecting Veterans with qualified RSPs, and provides rehabilitation information and recommendations to support VAC decision-making.
  • You are at the center of the rehabilitation process. PCVRS works closely with you, your RSPs, and your VAC Case Manager to ensure your Rehabilitation Plan addresses your program eligible health problems and supports your unique rehabilitation goals.
